There are 18 identifiers and includes patient records and information whether on paper or verbal and relates to the past, present, or future physical or mental health or condition of an individual. The provision of health care to an individual. The past, present or future payment for the provision of health care for an individual.

ePHI stands for Electronic Protected Heath Information and applies to the same 18 identifiers and statements above.
